There is only despair and no faith in God to relieve the despair. This is King Gurthrum's song which express the bleakness of his heart. He sings in reply to the songs of Ogier and Elf before him , that, by now men have grown up to realize that there is no comfort of truth in any of the fanciful tales of gods and youth. Is Language An Instinct Education Essay As you are reading these words, you are taking part in one of the wonders of the natural world. Linguists have continually been bewildered by language and the language debate which has been inundated with arguments by several professionals to support either the instinctive or learned side of the debate. So, in 1994 when The Language Instinct by psychologist Steven Pinker was published, it reignited the discussion. His book utilized the concepts developed by Chomsky who believed that language was instinctive due to a universal grammar- an innate design containing characteristics common to every human language. The other side of the argument builds on the theories by Karl Popper. Geoffrey Sampson (1997) and other linguists held the belief that language is developed by observing and learning from others because we are born with a blank slate. In this essay I will discuss both sides of the arguments in the hope of concluding whether or not language is an instinct. On the one hand, those who believe that language is an instinct express that language is not learned and does not depend on having had the best education. Linguistic ability is not learned like the way we learn to tell the time, or the way we learn to tie our shoelaces. Instead, it is a specialised and intricate skill which forms part of the brain, and develops in a child without conscious effort (Pinker, 2007). Behaviourists claim a childs imitation of their parents language initiates a childs language, yet there are examples of imitation which do not support this concept. Pinker (2007) uses the example that if children imitate parents then why is it that children do not imitate their parents quiet behaviour on airplanes? Chomsky (1980) produced an argument based on the poverty of the stimulus which stated that language is not learned because the information babies are exposed to is much less complex than the data and the rules they end up gaining. Therefore, it is doubtful that language is obtained through a method which consists of learning only. Instead his work suggests that a significant part of language is innate. It is important to consider the idea of universal grammar because if this is genuine and all human languages have aspects in common, then it is possible to say that language is instinctive (Schlenker, 2006). Chomsky supposed that language is innate because every language has a mutual structural basis since there is a part of the human brain which encompasses a limited set of rules, known as universal grammar, for managing language. Another argument to support the statement that language is an instinct is the idea that children invent language. Children may construct a new language, the rules of which seem to be established by that of universal grammar, when they have not been exposed to a clear and logical language. An example of this is the creation of creoles which are languages generated by children who have grown up in an environment involving the use of pidgins (languages that have progressed as a way for people to communicate when there is not a common language between them). Pinker (2007) expressed that in communities where the adults had conversed with a pidgin, the children who had only been exposed to this pidgin transformed it into a creole. The development of language by deaf children in Nicaragua is another similar example to illustrate the view that language is an instinct. In the 1980s when schools for deaf children were built, the children who first started attending the schools developed a meth od of communicating using signs, which was pidgin-like in nature. We can assume a dissociation between language and intelligence because even when intelligence is lessened language withstands. There are two particular cases which provide evidence for this and, in turn, the language instinct: when individuals have average intelligence but their language is significantly impaired; or, when individuals experience an impaired intelligence yet their language is normal. Brocas aphasics and Selective Language Impairment (SLI) patients provide evidence for the first example because they have a normal intelligence but experience extreme problems with their linguistic ability. Brocas aphasics specifically struggle with the production of language and comprehension, whereas SLI patients particularly face problems with the organisation of words into sentences (Expressive Aphasia, 2012). On the other hand, Williams syndrome patients are individuals suffering from an impaired intelligence but normal linguistics ability. Their language is even more developed than others of their age and they can converse with strangers at complete ease. However, they have a low intelligence due to abnormalities in parts of the brain including the cerebellum, right parietal cortex, and left frontal cortical areas (Williams Syndrome, 2012). Eve was not a born know-all. She was ignorant. But she was a good learner (Sampson, 1997). On the other side of the argument Geoffrey Sampson, and many others, for example, contemporary linguist Givon, believed that Pinker and Chomskys argument that language is purely instinctive is neither adequate nor plausible. The main belief expressed by Sampson (1997 or 2012) was that children can effectively learn languages because all individuals are good at learning anything that they are exposed to, it is not fixed structures in part of the brain which contain this in-built knowledge. Behaviourists vocalise that language is learned early in a childs life and consists of many complex systems. Although most children who are five years old have enough vocabulary to be able to communicate competently with others, there are individual differences between children in the capacity of their vocabulary (Blewitt, 2006). Research has found that language is linked to a childs home and school environment (Cunningham, Stanovich, West, 1994, as cited in Blewitt, 2006), and that the variety and amount of language the children are exposed to by conversations with their parents are linked to a childs vocabulary. In a study conducted in 1992 which was conducted over 2 years with visits made to children every month at their home. There were two conditions with participants from either poor families on benefits, lower middle class families (mainly occupying blue collar jobs), or upper middle class with at least one professional parent (Hart Risley, 1992, 1995, as cited in Blewitt, 2 006). All of the parents were actively engaged in playing with their children but the amount of verbal communication each group made with their child was different. In a week, consisting of 100 hours, a child with a professional parent hears 215,000 words but only 62,000 in the poorest homes. By the age of three, there was a positive correlation between the input of the parent and the language of the child. Furthermore, when the researchers looked at just one of the socioeconomic categories, therefore social class was not a factor in the result, the more language the child was exposed to, the more advanced the childs vocabulary. This provides strong support for the idea that language is learned rather than instinctive. John Locke provided the contrary view to naturists by claiming that experience is vital in the development of language. He states that a child is not born with knowledge and the concept of reason, but what is important as the child grows up is the exposure to language and so, it is by degrees he comes to be furnished with them (Sampson, 1997). This particular view which expresses that experience is the cause of all knowledge is known as empiricism. The clear objection to the view that language is instinctive instead of learned, as Chomsky and other naturists believed, is that we would expect everyone to grow up talking in the same language. However, people do not all speak the same language and the differences in the language people speak does not rely on our biological makeup. For example, if a newborn English baby grows up in Japan by Japanese speaking adoptive parents then they will become fluent in speaking Japanese speaker rather than fluent in speaking English. Without a language rich environment which nurturists see as essential for a child to develop language, a child will not acquire the capability to speak. Profoundly deaf children complete the first developmental stages towards speech such as babbling at the same time as those children who cannot hear. However, they rarely grow up into speakers because without the capability of hearing themselves or other people around them, they decrease the amountof babbling which rarely leads to word development (Kiel, 1998). In conclusion, the language debate has provoked much controversy amongst naturists and nurturists. Pinker and other naturists believe language is instinctive and their beliefs continue on from Darwins account that the gradual evolution of instincts generally by natural selection could be applied also to the human acquisition of the capacity for language () On the other hand, Sampson and other nurturists have found significant evidence to support the idea that children are born with blank slates and that it is by observation and imitation of parents and those around them that they develop the linguistic ability early in their lives until age 6-10 when children can converse effectively in challenging settings (Language Development, 2012). It is expected that a combination of an innate instinct to produce syntax with the imitation of the language of parents is the key to a child developing an extensive language.

Ranging from her state of mind in total isolation, her 'childhood', to her changing 'adolescence' and eventually reaching 'adulthood' and death, all in a sort of quick-motion. It will further deal with the development of tension throughout the poem. By making a distinction between tension through formal aspects, such as rhyme scheme, and tension through content it will try to show the interconnection between both of them. Additionally, the paper will deal with the possible effect of tension on the reader and how the poem might be perceived by him/her. 2.  Ã‚  Ã‚  Ã‚  Ã‚  Growth of Consciousness and Development of Tension 2.1.  Ã‚  Ã‚  Ã‚  Ã‚  Initial Isolation Lord Alfred Tennyson's poem starts out with the total seclusion of the beautiful, young Lady of Shalott surrounded by '…four grey walls, and four grey towers…';. (Tennyson, line 15). She is largely unaware of the world outside and of her own existence as a woman, only being able to see shadows of reality, which are reflected by her magic (?) crystal mirror and worked into her web. At this point of time it is unknown to the reader whether the Lady of Shalott is forced to be in this situation or chose to live this life of isolation. Reasons for a self inflicted seclusion might be homophobia, the fear of rejection by the exterior world, or simply a lack of interest for it. However, the Lady of Shalott is quite content with what she has and what she does. Her life of art in front of the loom and the crystal mirror is all she needs singing her ';…song that echoes cheerly…'; (30) throughout the land. She is as innocent as a careless child giving little thought about her future. The Lady of Shalott has no urge to leave her own interior world because she is not aware of any other options. Little is known about her outside '…many-towered Camelot.'; (5) and apparently she intends to keep it that way: But who hath seen her wave her hand ? Or at the casement seen her stand ? Or is she known in all the land, The Lady of Shalott ? (24-27) By now the reader might ask him-/herself why the Lady of Shalott is stuck in such a dreadful situation and why she does not attempt to do anything about it. There is a cons... ...martyrdom and so the story ends, leaving the reader in anger about the carelessly spoken words of Lancelot. 3  Ã‚  Ã‚  Ã‚  Ã‚  Conclusion   Ã‚  Ã‚  Ã‚  Ã‚  'The Lady of Shalott'; was and is subject for countless interpretation by readers and artists, especially pictorial. Although, or maybe because the whole setting is quite vague and shadowy there have been many attempts to explain why the Lady of Shalott was cursed. Maybe she was not cursed al all. What was this whisper she heard ? Was she schizophrenic, hearing voices inside her head ? Another question is why she had to die in the end. Is it possible she eventually commit suicide, because she had realized that her love for Sir Lancelot was futile ? These are some questions that stay in the back of your head after reading Tennyson's poem. Even today you can feel the infinite struggle the Lady of Shalott had to undergo. Tennyson impressively shows the development of a young woman who is faced with one of the most difficult times in life- growing up in a world full of restrictions and rules and becoming a woman. His subtle description of a girls problems at his time is, slightly altered, even valid today.

Their Eyes Were Watching God Literary Analysis Essay In 1936 when Zorn Neal Hurst first started to compose her award winning novel Their Eyes Were Watching God she deliberately fashioned the aforesaid work so that Its textual structure created anticipation amongst Its readers. She did this by including great adversity for the main character Jeanie to overcome. Jeanie became entangled in the oppressive powers of early 20th century marriage. That of which constrained her for the greater part of the novel.Going from man to man only intuiting the tyrannical cycle of being property. Throughout the course of the novel the reader wants Jeanie to find herself and break free. This creates anticipation within the reader. The reader was present during the beatings and the harassment Jeanie experienced. The only reason why the reader is in fact still reading is because of the anticipation he or she has building up inside of them. They only want the best for Jeanie and they want to be with her wh en she experiences it. All of the hardships and perils Jeanie experiences must lead up to something.It is through all the adversity hat Jeanie perseveres through that creates anticipation within the reader. Their Eyes Were Watching God was precisely articulated so that it naturally created a bit of hope Inside the reader. The reader wanted Jeanie to finally find love as well as find herself. This feeling amongst the reader first emerges when Jeanie is forced to marry Logan Clicks. â€Å"Jeanie and Logan got married in Nanny's parlor. † (Hurst 21)Jeanie just wants to have someone in her life, something to ward of the recurring loneliness she experiences. But even the reader knows that Logan Clicks is not the answer.This in itself creates anticipation. The reader at this point knows something will happen and may speculate what could occur. Essentially all that evolves out of this relationship Is Heartbreak for Jeanie. Her relationship with Logan soon dissolves despite the finan cial security he brought to the relationship. â€Å"Genie's first dream was dead, so she became a woman. † (Hurst 25) The reader knows that Genie's has partially given up on her dream of marriage but still wants her to find love. Fundamentally the whole relationship Itself with Logan Kills in turn creates anticipation amongst the reader.All of which plays a role In the reader's hope that Jeanie will find love and â€Å"all will be well In the kingdom. † In a second sequence of events Jeanie finds herself In a relationship with the ever so powerful mayor, Jody. This marriage is the foundation for more heartbreak but also more hope for the reader. It all began when Jody â€Å"slapped Jeanie until she had a ringing in her ears. † (Hurst 72) That is the turning point in their relationship. Jody had been an overbearing figure in the relationship by limiting Genie's social contact with the outside world, but never had he been oppressive In a pugnacious manner.The read er feels for Jeanie at this point. He or she more anticipation is created amongst the reader. The reader knows that something will happen further down the road. That the previous order of events foreshadows something greater. No situation Jeanie has experienced in the past has remained constant. It seems adversity is inevitable for Jeanie. This forces the reader to ponder the many possible outcomes of Jeanie and Jody relationship as well as Genie's quest for love itself. The aforesaid inevitable adversity that Jeanie experiences will once again come into play as Jeanie engages in a new relationship with Teacake.Jeanie awoke next morning by feeling Tea Cake almost kissing her breath away. Holding her and caressing her as if he feared she might escape his grasp and fly away. † (Hurst 107) The reader at this point is relaxed finally Jeanie has found a man that treats her like a woman. That respects her and seems to love her with all his heart. With all the different men that Jean ie has been with the quality that Teacake has that sets him apart from the other ones is compassion. The reader for the first time experiences one of the qualities of true love alongside of Jeanie. The novel might as well be over at his point all is well.But then again there is that everlasting feeling in the mind of the reader. Jeanie has never had everything go as planned. The reader anticipates something to go wrong. The past is ever present during Tea Cake's relationship with Jeanie when Jeanie â€Å"found out her 200 dollars was gone. † (Huston 118) Jeanie soon discovers that Teacake is also gone. This order of events creates immense anticipation in the mind of the reader. He or she asks him or herself questions like: â€Å"Did Teacake really steal her money and leave† or â€Å"will Jeanie Just finally give up on her ream of finding love. All of this anticipation and speculation on the reader's part was precisely planned by Zorn Neal Hurst to keep the reader inte rested in her novel. In her 1937 masterpiece Their Eyes Were Watching God, Zorn Neal Hurst creates great anticipation amongst the reader through her use of textual structure. By precisely ordering events Hurst is able to effectively create said anticipation. By incorporating the ever apparent theme of Genie's quest for love, into her order of events Hurst creates much anticipation amongst the reader.

The following questions that I would like to ask are: how do people get over their love ones death? Are there ways to overcome it? Is this situation a bit too bizarre? Can we accept death? And what do you think about this issue? A similar topic is on a short-story that was created by Richard Selzer called, â€Å"Whither Thou Goest†, which explores how a wife was struggling to find happiness and independence due to her husband’s death, but eventually has the strength to move on with her life. Richard Selzer explains many opportunities to think about those large questions with some additional complications, since the story is not dealing with the burial whose heart is still beating, but the burial of a man whose heart is still beating in someone else’s body. â€Å"Whither Thou Goest† explains how a wife named Hannah lost her husband, Sam, who was killed in a shooting incident by an unknown stranger. As soon as her husband died, she quickly decided to donate all of his organs to others who are in need for transplants. Later in the story, Hannah became obsessed on trying to meet the person who received her husband’s heart, whose name was Mr. Pope, and listen to the heart for approximately one hour. She felt that the only way to reconnect with her deceased husband again was to listen to his heart one last time and her reasons to search for the heart were â€Å"A heart can be listened to. A heart can be felt† (Selzer 231), so instantly she started to write letters to Mr. Pope. Hannah’s best friend, Ivy Lou, who appears to be the only one trying to help her to get over Sam’s death, argued that Hannah shouldn’t be contacting the recipient in the first place and to move on from her husband’s dea th even though she gave Hannah the recipient’s personal information, since she works for the Aetna Casualty and Life Insurance Company (Selzer 238). Eventually, Hannah achieved her goal by finding Mr. Pope who had received the heart, which the recipient ended up becoming her co-conspirator and protector of her husband’s heart. In my opinion, I wouldn’t say that Hannah was going through a bizarre situation, but I would say that she could have dealt with her husband’s death in a reasonable manner. Some ways could have been therapy, spiritual counseling, new occupation, traveling, education, a new love interest, or other positive activities. The only thing that I didn’t understand is why she donated his organs. I’m not sure if she wasn’t really thinking or hadn’t thought about it ahead or didn’t really know how she was going to feel afterwards, but I felt that she could had kept his organs and possibility start to visit his grave when she wanted closure. While constantly reading this story, I suddenly realized that I also had my own share on finding a way to cope with the loss of a family member. I remember when my uncle, Red, died from a heart attack in 1994. I felt nothing but sorrow, grief, and was even depressed about his death. I even couldn’t get closure from it nor talk about it to someone since everything was happening too fast. The main reason why I was upset because no one in my family told me until two years later that he had passed away and it was painful and shocking to hear that, since he was like a father to me and was the only person in my family that I could relate to; somehow I ended up finding a way to get closure. So instantly I would sit in his bedroom and do similar things that he used to do like eat his favorite foods, read his collection of bibles, or even watch some of his favorite TV shows. I used to stay in his room for days and hours while never leaving his bedroom. Within a year, I finally realized that I did achieve my goal of getting closure from his death and never went to his bedroom ever since then and started on the process to move forward with my life. So I don’t think what Hannah’s going through is not bizarre, but I feel that everyone hates to see people die or sometimes face things alone. The main point is that we can’t accept death. Sometimes we don’t realize that someday we will all dead; but when you lose someone who was a close friend, a spouse, or family member it hits you hard and just knock you off your feet. Within time it allows us to heal from a terrible lost while time takes away the pain. It’s a slow process that only time will allow us to heal. There is no one that can tell you a time limit, because there is no limit, but soon or later you will move on with your life and one day we will have to face daily issues without anxiety, fear, and being able to accept death. They may be in fear of their life because crowds feed off of each other and can become very aggressive and violent. In some instances the officer will face gunfire and this puts the officer in a life or death situation. The officer must adapt and control the situation and use his or her training to get the facts and information from the bystanders or the victim(s). With all the challenges and stressors that come with policing there are also satisfactions that come with the job. Officers serve and protect the public. They patrol the streets to ensure the community is safe from would be offenders or to apprehend the ones that have already committed crimes. They take individuals of the streets that are a danger to society. They help people that may have a flat tire, domestic disputes, or have had their home broken in to. The officers feel satisfied when they capture the suspect and when they help people in need. When the officers are recognized for their good work they are satisfied with their job and may become more motivated to improve their performance on the job. Many strive to climb the ranks and with each promotion come more satisfaction. A career in law enforcement in itself is a satisfaction that many people do not get. Many officers do not do the job for the pay but rather the rewards that come with the job in serving and helping the community. Certain officers find it very rewarding when they can help a juvenile down the right path and to possibly mentor others from making life changing mistakes. Officers need to stay positive in their work and duties to ensure job satisfaction because it can affect the work they perform and also affect community relations as to how they see the police. Thus, poor performance can impact police-community relations by adversely affecting public attitudes toward the police (Buzawa, Austin, Bannon, 1994). In the routine of having a positive work attitude and environment it creates less stress levels and fewer symptoms of stress that can lead to absenteeism, burnout, and alcoholism. There are many reasons why people do not want to become part of the law enforcement community but there are a lot of reasons and rewards why other people feel they are being called to serve and protect. Social media has become both a window and a bridge to the public relations practitioners, by enabling them to interconnect the internal objectives of the organization with the constant changing externalities. In the arena of public relations, an array of hats is essential to effectively carry the overall message of an organization into a stage of visible outcome. An organizations public relations department amongst many other tasks must build awareness, research and analyze public attitudes and perceptions, strategize, and implement crisis plans. Social media provides a way for organizations to do these things on a scale not known in earlier times when messages had to first bypass the filter of the media gatekeepers. As stated by Rupert Murdoch, Technology is shifting the power away from the editors, the publishers, the establishment, and the media elite. Now its the people who are in control (p. 1). Social media as defined by Wikipedia is an umbrella term for the various activities that integrate technology, and social interaction (p. 1). This includes internet forums, weblogs, social blogs, wikis, podcasts, pictures, and videos. Forrester, The Growth of Social Technology Adoption estimated in 2008, that 3 out of 4 Americans use social technology. It is this type of growth that makes it an inevitable, essential tool in creating a survivable existence. In the world of public relations, building awareness and brand reputation are cohesive. Social media is not only being used for its direct to audience approach, but also as an effective generator of media relations. Both awareness and reputation come on the tail of an organizations integrity. In order for an organization to build a positive reputation, it must nurture relationships. It is not science that viable relationships come from a foundation of trust. The trust that social media provides is authenticated by the fact that it allows for the relationship to be carried out in a two-way process. This type of communication permits an organization to truly listen to their publics needs and opinions, giving the organization an open opportunity to be actively responsive. Before social media, the standard press release into traditional industrial media was a shot in the dark towards reaching the true potential of interest. It is important to understand the many different branches to which an organization may utilize in building awareness and how. Through internet forums or social networking sites, such as MySpace, Facebook, and LinkedIn, to name a few, as well as blogs, make an organization able to centralize their audience. Instead of talking at the masses, they are able to talk with a selected tuned in, interested community. It is in these communities where a considerable buzz of web chatter stirred, enters the web world and reaches the eyes and mouths of influential mediums that were otherwise unlikely to be reached by just any Joe before. In social media, ideas can spread like a wild fire, through an effect called push and pull. According to Oden 2009, in the push aspect there is instigation of a new concept in social media situations, providing useful information and provoking discussions about it (p. 2). This creates an influence of pull which is the drive of traffic from people searching for more information. A common threaded resource between the push and pull theory is what is called Really Simple Syndication (RSS). RSS is a jeweled technology in the search and sift element of both public and media relations. On the side of the public relations, RSS is useful in recognizing content analysis, by keywords or concept references. This type of insight gives public relations a heads up into their competition and identifies their own strengths and weaknesses accordingly. On the side of media relations, journalists are able to receive content that is in relevance to their interests or beat. The research that takes place within the public relations department is essentially the bones of the overall success of an organization. It is a stage that is unending, and social media provides a platform for which information is delivered in real time format. Another dynamic of research affected by social media is the scale upon which surveys are able to be given. Not only do social media make it possible for samples to be gained on a global level, through certain software, presentations are part of digital exhibits and interactive sites record message resonation. Although, some may view this open door to the world mentality overwhelming, it is a way for an organization to see where their message may penetrate beyond their eyes conception. After the research, an organization must take all those pieces of information to organize, strategize, and implement into a plan. In the text Using Public Relations Research by Glen Broom and David Dozier (2009) Strategic planning is deciding where you want to be in the future and how to get there. It sets the organizations direction proactively, avoiding drift and routine repetition of activities. The quality of content is a core value in success, so developing a strategy that engages people is important. When formulating a strategy through social media the five Ws serve as an outline. Who are you trying to reach? What are you trying to accomplish? Where are these people online? When and how is the best time to reach them? Why will they care about what you have to say? In order to create a credible presence, the content must be consistent. Tactics that reinforce the strategy of an organizations power in the clutter of information is redundancy. Using multiple communication channels within social media, such as blogs and podcasts, fights entropy. Also, increasing the channels a message is delivered optimizes the search engine visibility. It is through search engine optimization that an organization leaves its footprints in the virtual world. However, these virtual footprints can have adverse affects in the light of a crisis. A constant flow of information circulating can lead to misinformation, and in turn misunderstanding (McLintic 2009). A good crisis management plan must involve actively monitoring the web for any content that could be damaging to the organizations image and reputation. Social media gives organizations a way to be transparent in their management. With a constant flow of communication between an organization and its public, it is much easier to indentify and diffuse a small problem that couldve escalated to a bigger one (Hall 2009). As with any situation, it is ideal to be proactive rather than reactive in nature. There are certain methods of preparation that can greatly change the outcome of a crisis. Through anticipation, an organization can take a front line approach to crisis planning. One way that a crisis can be redirected, is by the purchase of generic crisis terms that ensure search engine positioning of the public to landing pages set in place to address issues, before they link to others that could possibly reinforce negativity. Before venturing off deep into the social media dimension, it is a good idea to benchmark the current position of the organization, so that there is a clear start of measure on the ROI. A few simple markers Chris Lake suggests in his article 10 Ways to Measure Social Media Success would be to make a note of the obvious numbers, (facebook fans, twitter followers, dig links, etc). Make a note of the less obvious benchmarks, (SEO rankings, customer satisfaction scores). Make a note of ROI benchmarks, such as how much is being paid to acquire customers via other marketing channels. However, reality is, social media humanizes a process that was once broken down so analytically. In order to measure the success of social media, you must come to the realization that there is no true assessment by which impact can be calculated. Although, it should not go without saying that it stands as an effective means to communicate to millions, far below the cost of any other form of media. The relationship it assists in building is priceless, and retention is a pretty effective bottom line tool of evaluation. In every new media there has been resistance to change. As a product of generation Y, I have been witness to the crossover over in media convergence, and I see a world of possibilities as it evolves. In the world of PR, I feel social media is an undeniable asset to an organizations ability to connect in ways that attributes to immeasurable value in what they strive to represent and stand for. Social media is a hub to the PR obligations. To underestimate its power to redefine and shift the foundation, by which PR once stood upon as news beggars, is to remain oblivious to the treasure bestowed at our fingertips.

The Political Writings of Charles Brockden Brown Charles Brockden Brown, famous as the first professional American writer, was an inventive creator of novels, stories, pamphlets and journal articles. His life extended from 1771-1810, over some of the most significant periods of American history. He came from a Quaker community of Philadelphia, a very intellectually and politically active city. Thomas Jefferson: Contributions to Literature and Other Fields :: essays research papers fc

Thomas Jefferson is commonly most notable for his contributions to American political history. He was President of the United States, the first secretary of state to George Washington, minister to France with Benjamin Franklin, governor of Virginia, and congressman. (725) Jefferson’s literary works strongly reflect the focus, love, and ambition that he had for this country. He was born unto Jane Randolph Jefferson and Peter Jefferson. His mother was from a wealthy, and one of the first families of Virginia. His father was a self-educated man that became a country official and surveyor. (725) Sadly, Peter Jefferson died when young Thomas was only fourteen leaving him two thousand seven hundred and fifty acres of land.   Ã‚  Ã‚  Ã‚  Ã‚  Jefferson entered the college of William and Mary in 1760 when he was but seventeen years old. His habits were those of patience and severe application. Math was his favorite study, at which he very much excelled in. When came time for relaxation, he exercised his skill at the violin. Jefferson swiftly graduated in only two years with the highest of honors that the school offered. Afterwards, he studied law with a friend from William and Mary, George Wythe. In 1769 he began six years of service as a representative in the Virginia House of Burgesses. The following year he began building Monticello on part of the land inherited from his father. The exquisite mansion, which he designed in every detail, took years to complete, but part of it was ready for occupancy when he married Martha Wayles Skelton on January 1, 1772. They had six children, only two of whom survived into adulthood. Virtue theory not only concentrates on how an individual acts but also what a person should strive to be, for example a religious figure may personify perfection when in the public when they really should strive for that perfection at all times. The deontological theory states that people â€Å"have a moral obligation to follow certain principles. I liken this to the Ten Commandments, rules or principles that we all are expected to follow. â€Å"The utilitarian theories, as supported by John Stuart Mill, call for generating the greatest aggregate good for the greatest number of people. One major benefit of such theories is that they take consequences into account. They seek specifically to promote the human good as a whole. They also provide guidance for behavior, enabling people to know what qualifies as the moral choice. † I will again use the religious area as an example because to me this is what the religious leaders do.They preach to the multitudes to be good and do good and they also offer guidance. The differences between virtue theory, utilitarian and decontological are that â€Å"virtue ethics cannot generate specific rules to guide behavior. † Under the utilitarian theory â€Å"only total human good or happiness matters. † Deontological theories â€Å"do not always clarify how to rank duties. † I lived in a very small community where everyone knew everyone and we all thought we were living the life of good moral standards and values, until one day I saw a very prominent member of the community, enter an establishment that everyone knew was a hangout for drug users.This person was there for a long time and when they came out you could tell they were under the influ ence. 6 steps decision making Essay

In addition to time pressures that we encounter when searching for evidence to support care decisions for individual patients, it may be difficult for clinicians to apply the evidence that we find. The rate limiting step may not be doing the search, but the steps needed in â€Å"setting evidence-based medicine (EBM) in motion.† We present an example of a search for evidence by a Physician Assistant (PA) student that highlights this challenge. PAs receive accelerated training in the medical model and work in teams  under physician supervision. Approximately 40 000 PAs currently work in the  US in a wide range of settings and specialties.  Practising EBM has become an important component of training for PAs.  During an internal medicine rotation, a PA student encountered a common clinical practice unsupported by current  evidence—administration of nebulised albuterol in patients with community acquired pneumonia (CAP). While this practice may  be justified in patients with underlying chronic obstructive pulmonary disease (COPD) who also present with CAP, this student  questioned the grounds for its use in patients with CAP who do not have COPD. Clinical scenario A 68 year old man presented to the emergency department with fever, chills, and a non-productive cough of 1 week’s duration. He had fatigue, headache, rhinorrhoea, and mild nausea, but  denied dyspnoea. He had no history of smoking or COPD. He  had atrial fibrillation and was taking warfarin for stroke prevention. On admission, his temperature was 38.4  °C, heart rate was  108 beats/minute, respiratory rate was 24 breaths/minute,  blood pressure was 156/88 mm Hg, and oxygen saturation was  86% by pulse oximetry on room air. Rales were heard in both  lung bases and in the right middle lobe. Chest radiography  showed a diffuse infiltrate in the right middle and lower lobes. Complete blood count showed a white blood cell count of  22 000 cells/ml with a left shift, and arterial blood gases showed mild respiratory acidosis.  One dose of ceftriaxone was administered parenterally, and a course of azithromycin was started. Albuterol, 5% solution,  delivered by n ebuliser 3 times daily was also ordered, in addition to a combination of inhaled ipratropium and albuterol,  delivered by metered dose inhaler every 4 hours as needed.  During the hospital stay, his pneumonia resolved, but his heart rate increased to 150 beats/minute and his blood pressure rose from 156/88 to 200/110 mm Hg. Clinical question Although there was no institutional protocol for use of  nebulised albuterol for treatment of CAP, the house staff often ordered it.  The PA student queried: In a 68 year old man with CAP and no underlying COPD, does use of nebulised  2 agonists improve symptoms? What is the risk of harm in this  patient? Search strategy Firstly, a treatment guideline was sought to clarify recommendations regarding use of nebulised albuterol for treatment of CAP.  The American Thoracic Society guidelines for management of  CAP1 were rapidly retrieved through PubMed, UpToDate, and  MD Consult. The British Thoracic Society (BTS) guidelines for the management of CAP in adults2 were also found in PubMed.  Both sets of guidelines were relevant to our patient, but neither guideline discussed the use of nebulised albuterol in the  treatment of CAP. The BTS guidelines had a section on general management, which discussed the use of adjunctive therapies  for CAP, but nebulised albuterol was not mentioned. Evidence from controlled clinical trials was mentioned in the guideline for â€Å"bottle blowing,†3 but not for physiotherapy.  Having not fully answered our question with a review of  relevant guidelines (and having not attracted the attention of anyone who could change the patient’s treatment p lan), we  searched PubMed again, this time specifically for studies on the use of albuterol in patients with CAP. No relevant trials were found on the use of nebulisers for CAP.  To identify evidence about harm with the use of albuterol,  PubMed was searched using the terms nebulised albuterol, cardiac arrhythmias, and randomised or controlled clinical trials.  No trials were found. When just the content terms were  searched, 9 articles, not directly relevant to our patient, were found. One prospective, open label study on the effect of  nebulised albuterol (for treatment of asthma) on cardiac rhythm was found.4 10 patients were studied, and although no adverse effect on cardiac rhythm or blood pressure was found, the study did not convince the team that no potential for harm existed in this, or other patients, especially when there  was no clear indication for use of albuterol. Recognising that searching and appraising the literature are not the only important aspects of practicing EBM, we consulted an experienced pulmonologist, who practises and teaches using the EBM model. In addition to reviewing treatment plans for  multiple cases of CAP requiring hospital admission with the  Nurse Practitioner/Physician Assistant service, he recommended review of the Centre for Evidence-Based Medicine  website at Mount Sinai Hospital in Toronto, Ontario, Canada  (www.cebm.utoronto.ca/), which suggested bubble blowing as a method for helping clear secretions.2–3 This served as an  excellent, rapid approach to finding good information on treatment of CAP, and confirmed the evidence previously found in  the literature search.  Application of the evidence to this, and future  patients  The treatment plan for this patient was not altered by the  student’s rapid search for evidence. Changes in usual care for a common illness required a compr ehensive search and discussion among all clinicians in our institution caring for patients  with CAP. The clinical team reviewed the results of the search and because no evidence was found to support use of albuterol in patients like ours, changes were made to future practice. As a result of this process, which took a few hours and evolved over several weeks, orders for bronchodilators for patients with CAP are now made on an individual basis, depending on the  presence of patient comorbid illnesses, such as COPD. 164 Volume 8 November/December 2003 EBM www.evidence-basedmedicine.com EBM notebook Downloaded from ebm.bmj.com on 10 August 2009 Conclusion The need for a rapid search for evidence is sometimes, but not always, important to the care of an individual patient. In this case, the speed of the search did not affect the ability of the PA student to apply the  evidence to the patient. Setting the evidence in motion may require communication of search results to other members of the clinical team and may affect the care of future patients. Although the catalyst for setting EBM in motion was a student, the evidence, including the results of further research, along with the judgment of the experienced pulmonologist,  convinced the clinical team to make changes to usual care and to base future treatment of this common condition on the best available evidence. The basic concept of evidence-based medicine proposes to  make health related decisions based on a synthesis of internal and external evidence. Internal evidence is composed of knowledge acquired through formal education and training, general  experience accumulated from daily practice, and specific  experience gained from an individual clinician-patient relationship. External evidence is accessible information from research.  It is the explicit use of valid external evidence (eg, randomised controlled trials) combined with the prevailing internal evidence that defines a clinical decision as â€Å"evidence-based.† To realise this concept in day to day clinical practice, the Evidence- Based Medicine Working Group proposed a 5 step strategy,  corresponding to step 1 and steps 3 to 6 shown in the left hand column of the table. In teaching this 5 step approach, we encountered several difficulties. We noticed a growing hesitance to accept this strategy  as students advanced in their medical training. In the presence of well established methods of treatment or diagnosis, this  resistance rises even more, regardless of the level of training.We assume that this barrier is associated with the process of socialisation into the health professions. Throughout medical  education students are virtually â€Å"trained† to make decisions under the condition of uncertainty. Advanced students and to a greater extent clinicians lose some of their ability to differentiate between scientific evidence and what seems to be evident. If we intend to implement evidence-based medicine more efficiently, we need to modify the way students and clinicians learn to make decisions. Therefore, an additional step was introduced in our evidencebased medicine teaching programme (step 2 in the table).  Students were to provide answers to their clinical questions based on their current knowledge (internal evidence) before  continuing with the remaining steps of the evidence-based  process.2 Our collective experience concerning this additional step was extremely positive. The students using this new step were satisfied that their pre-existing knowledge had been  integrated into the evidence-based approach. By explicitly  documenting their internal evidence, students used the remaining steps of the process to evaluate not only the best evidence in making a clinical decision but also to assess the accuracy of their internal evidence, the grounds upon which their preconcep- The 6 steps of evidence-based decision making Step Action Explanation  1 Transformation of the clinical problem into 3 or 4 part question (a) relevant patient characteristics and problem(s), (b) leading intervention, (c) alternative intervention, (d) clinical outcomes or goals. 2 Additional step: answer to the question based on â€Å"internal evidence† only Internal evidence: acquired knowledge through professional training and experience (in general and applied to the patient). (or is my patient so different from those in the study that the results do not apply?) 5 Integrating external and internal evidence The 2 sources of information (external and internal) may be supportive, non-supportive, or conflicting. How the decision is made when non-supportive or conflicting will depend on multiple factors. 6 Evaluation of decision making process Once the decision has been made, the process and the outcome are considered and opportunities for improvement are identified. The health authority of Alto Adige in northern Italy initiated and supported a project, the â€Å"Bressanone Model,† in which the effects of implementing evidence-based medicine on the quality of health care were to be shown. In this model we used the six step approach, which proved to be successful in the student  project to teach experienced clinicians.3 The participants were asked to name problems of their day to day practice that lacked either an effective or an efficient solution. The evidence-based medicine support group helped participants to phrase the 3 or 4 part questions. Subsequently, the physicians were asked to submit their individual answers to the questions before continuing with steps 3 to 6. Agreement between internal and external evidence varies. Completing the full process could result in finding evidence that confirms the internal evidence, validating and strengthening the clinician’s or student’s confidence in the decision. The process could also reveal that little evidence exists to support the decision or that the available evidence is equivocal. In such cases, other factors such as cost or inconvenience to the patient may need to be given greater consideration. Possibly, the best external evidence found is not in agreement with the internal evidence. This represents a particularly valuable experience for the clinician or student because it may avoid an ill advised decision. It also shows the fallibility of making decisions on  uncertain ground based on internal evidence alone. This in turn will hopefully promote the routine assimilation of external evidence in clinical decision making. The documentation and  comparison of steps 2 and 5, used as a research tool or quality assurance outcome measure, coul d provide valid information  on the effects of evidence-based medicine on clinical decision making. In case of conflicting internal and external evidence, clinicians have several options. They may change their mind and align it with the external evidence. They may determine that the external evidence is not sufficiently convincing and remain with the  original decision. Or, they may choose to discuss with the  patient the conflict between the internal and external evidence in a manner that enables the patient to take part in the decision making process. This last approach is recommended because  patient preference is considered an essential part of the  evidence-based decision making process1 and decisions often  need to be made in the absence of clear research findings.

History of Colombia

History of Colombia Colombia was inhabited by indigenous societies during what was known as the pre-Columbian period (â€Å"Background Note: Colombia,† 2012). These indigenous societies consisted of hunters and nomadic farmers, as well as the Chibchas (â€Å"Background Note: Colombia,† 2012). The Chibchas are known to be one of the most developed indigenous groups in all of South America (â€Å"Background Note: Colombia,† 2012). Santa Marta was founded in 1525, which was the first permanent Spanish settlement (â€Å"Background Note: Colombia,† 2012).Santa Fe de Bogota was founded in 1538, and in 1717 it became the capital of the Viceroyalty of New Granada (â€Å"Background Note: Colombia,† 2012). The capital of New Granada is what is now Colombia, Venezuela, Ecuador, and Panama (â€Å"Background Note: Colombia,† 2012). In 1808 Napoleon made his brother the new king of Spain and people in the Spanish colonies were not happy with it (Lambert, 2 011). Citizens of Bogota created the first representative council against Spanish authority on July 20, 1810 (â€Å"Background Note: Colombia,† 2012). In 1813 they got complete independence from Spain (â€Å"Background Note: Colombia,† 2012).In 1819 Simon Bolivar defeated the Spanish in the war of Boyaca (Lambert, 2011). In 1819 the Republic of Greater Colombia was created and it included everything of the previous Viceroyalty of New Granada (â€Å"Background Note: Colombia,† 2012). Simon Bolivar was the first president and Francisco de Paula Santander was elected as vice president (â€Å"Background Note: Colombia,† 2012). The creation of the two main political parties, Liberal and Conservative, were led by conflicts going on between the followers of Bolivar and Santander (â€Å"Background Note: Colombia,† 2012).Bolivar's supporters formed the Conservative Party, and Santander's followers created the Liberal party (â€Å"Background Note: Colombia, † 2012). Colombia's history is known to be filled with violent conflict (â€Å"Background Note: Colombia,† 2012). Two civil wars were the cause of rivalry between the Conservative and Liberal parties: The War of a Thousand Days which took place from 1899 to 1903 and killed about 100,000 people, and â€Å"La Violencia† (the Violence) which took place from 1946 to 1957 and caused about 300,000 deaths (â€Å"Background Note: Colombia,† 2012).From the 1960s to 1980s the guerillas and cocaine production in Colombia started to increase (Lambert, 2011). The drug trade also caused a lot of violence (Lambert, 2011). However, at the start of the 21st century the situation in Colombia had improved after suffering a great depression in the 1980s (Lambert, 2011).
